I have a wintv ci usb, but the option external ci is grayed and can't select.

That is with DVDViewer 5.4 beta. How I can enable the option?

It is only for the old Hauppauge WinTV-CI/TerraTec Cinergy CI USB if such a device is installed the option should be enabled.
